Issuing of death certificates

The Department of Home Affairs will issue a Death Certificate on receipt of the notification of death (Form BI-1663)and the Death Report (Form BI-1680).

This includes a spouse, siblings, and children. But death certificates can be requested by anyone when they become public record. In some states, death certificates are released 25 or more years after death. Check with your state's vital records office to find out when death certificates become public record.

The Medical Examiner's Office (MEO) does not provide death certificates. To get a death certificate, contact the Pennsylvania Department of Health. To request records having to do with an autopsy or investigation done by the MEO, you must fill out the records request form and pay for the reports you want.
